Sahaya Therapy, LLC accepts a range of insurance, EAP, and self-pay options to make therapy accessible and convenient.
We currently participate with most PPO and major Employee Assistance Programs (EAPs).

Payment Methods : We accept credit cards for all Sahaya Therapy services, including co-pays and other fees.

Pre-Registration Process

The pre-registration process is complete only when the Medical Insurance, Demographic data and HIPAA, Consent Forms are electronically read and signed.

• The Administrative staff collects the HIPAA, Consent forms, Insurance/EAP information, payment information, insurance verification is processed and notified.

• Appointment scheduling is done after the Billing Specialist verifies the insurance benefits and pre-registration information. The billing specialist will email the client to inform them of their financial responsibility before the appointment.

• The scheduler sends an email verification to schedule the client with dates and times available with the chosen therapist.

EAP Pre-Registration

• Complete the Pre-registration information, HIPAA and Consent Forms, payment information, Insurance card JPG image, EAP authorization and all other demographic information before scheduling your first session.

Appointment Scheduling & Rescheduling

• Self-Pay: Clients that are not covered by insurance accepted by our practice should select the Self-Pay option

• Pre-registration, registration, scheduling and Billing assistance please email the contact information below.

• Rescheduling appointments will be done only via email. Please refrain from texts.

Self-Pay Clients

Clients who do not have insurance coverage, or whose insurance carriers are not on our accepted list, may choose the Self-Pay option.

Insurance Clients

If you are using insurance, please email a clear copy of both the front and back of your insurance card (in .jpg format) to Anita.Rock@sahayatherapy.com.

Before beginning therapy, all clients must review and complete the following forms to ensure a secure, compliant, and efficient start to care. These forms can be filled out electronically through our website or sent securely via encrypted email.

  • HIPAA Privacy Practices Form – outlines your rights and how your information is protected.
  • New Client Intake Form – gathers essential background and demographic information for your EMR.
  • Telehealth Psychotherapy Consent – confirms your consent to participate in HIPAA-compliant video sessions.
  • Group Therapy Informed Consent (if applicable) – explains participation expectations and confidentiality.
  • Payment Authorization Form – secures your preferred payment method for co-pays and session fees.
